Claims Process
Please take a moment to read important information about the claims process when using the First Health Network.
FOLLOW THESE EASY STEPS
- Present the card to your medical provider
- Provider will usually file a claim or member may file a claim directly
- Claim will be re-priced
- Carrier will pay the benefits accordingly
- Member is responsible for balance due (less contracted discount amount)
HOW TO FILE A CLAIM
- ELECTRONICALLY - For quickest payment, the provider may file electronically through Emdeon Payor #314177100
- MAIL CLAIM - Mail paper claim to the address on the back of the ID card. Following information is required:
- The member ID or a copy of the ID card.
- An original bill for services provided to include the diagnosis, CPT code, and date of service.
- Provider name, Tax ID Number, and address.
- UB92 form required if services were rendered in a hospital.
