Plan Benefit Services, Inc. receives information about our customers in order to provide customer service, offer new products or services, administer our products, and fulfill other legal and regulatory requirements. • Information provided by the employer and the employee on applications, forms, and surveys, such as  name, address, date of birth, social security number, gender, marital status, and dependents.
• Information about transactions and experiences with Plan Benefit Services, Inc. such as: products or services purchased, account balances, payment history, policy, coverage, and premiums.

Information Shared Within the Plan Benefit Services, Inc. Family of Companies
